These Terms and Conditions govern the provision of sports therapy services by Matt Tristram Sports Therapy. By booking or attending an appointment, you "the patient" agree to these terms

1. Professional Services

Matt Tristram Sports Therapy provides degree-qualified sports therapy and manual therapy. Clients must provide accurate medical information to ensure appropriate care. All treatment outcomes vary, and therapy is tailored to individual assessments conducted by therapist. Patients are also advised to follow the treatment plan provided. By attending the appointment you consent to assessment and treatment. You may withdraw your consent at any time. You may be required to remove items of clothing as appropriate to the treatment provided. We are fully DBS certified and patients under the age of 16 will require an adult to attend the full session.

2. Appointments and Bookings

Initial consultations last 45 minutes; follow-up appointments are 30 minutes. Clients are advised to arrive 5 minutes early. Late arrival might result in shorter session duration to ensure timely service for subsequent patients.

Appointments must be booked in advance.

Appointments are usually held at Renaissance Fitness, 12 Bye Street, Ledbury, HR8 2AA.

Please note the clinic is held in a private room located in the Gym. Limited parking on site. However three council car parks are situated local to the premises.

3. Cancellation Policy

A 24-hour notice is required for cancellations or rescheduling. Missed appointments or cancellations with less than 24 hours' notice will incur a cancellation fee equivalent to the full session rate.

4. Confidentiality and Data

All patient records are kept confidential in a cloud based system called Cliniko and handled in accordance with applicable data protection laws applied by clinic staff.

5. Liability

All treatments are carried out with care and professionalism. We are not liable for any injury or condition arising where relevant medical information has not been fully disclosed. Results may vary between individuals.
